Freedom of Information Requests (FOI) are handled by the Administration Department. The first ten (10) pages are free of charge and the remaining pages are $.20 per page, per action taken by the Village Board of Trustees on July 21, 1994, for copies of information or documents requested. This form can be mailed or brought to the Lincolnwood Village Hall, 6900 N. Lincoln Ave., Lincolnwood, IL 60712, Attn: FOI Request - Administration. ORIGINAL ONLY - IT CANNOT BE ACCEPTED VIA FAX.
